If you require any vaccinations relating to foreign travel please complete the online travel health questionnaire, which can be found further down this page. This should include the countries and the areas within countries that you are visiting to determine what vaccinations are required.
There is further information about countries and vaccinations required on the links below
It is important to submit this questionnaire as early as possible - at least 6 weeks before you travel.
The Nurse will review your questionnaire and then the Surgery will call you to make a convenient travel health appointment.
Your travel health appointment needs to be at least two weeks before you travel, to allow time for any vaccines to work. The nurse will provide travel health advice, information and the following vaccines free of charge (if needed)
- Hepatitis A
- Diptheria, Tetanus & Polio
Please note: There is currently a national shortage of Hepatitis A and Typhoid vaccinations. The Surgery will only be able to administer these vaccines if they are in stock. This is beyond our control
