Childhood (4-11) Asthma control test

Use this service to complete a childhood asthma control test.

You can use this service if:

• your child is aged 4 to 11 years
• your child is registered at the surgery

Before you start

We’ll ask you for:

• your details and your child’s details (name, date of birth, sex, postcode, email and phone number)
• answers to the Asthma Control Test, including questions your child should answer themselves
• information about symptoms over the last 4 weeks
• your child’s height, weight and (if available) peak flow reading
• details about inhalers and asthma action plans
