Asthma review (adults)

Use this service to submit a routine review of your asthma.

All children under the age of 16 require a face to face appointment with the asthma nurse, do not complete this form for children.

You can use this service if the patient:

• is registered at the surgery
• has been invited to do so
• is over the age of 16

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
